The temple is located at Muzhakoth on Kayyur road, 4 km from Cheruvathur in Kasaragod district.
Muzhakoth meaning "Muzhakunnath" was the old land name. Muzhakkom field was a green field. On the southern side of this beautiful field is the abode of Lord Sri Parameswar, the three-eyed Lord who showered blessings on the villagers. It has been centuries since he became famous as Muzhakothappan and became the savior of the country.This holy temple was once under the rule of Thazhakkat Mana. Brahmashree Mekkat Kesava Pattery is currently managing the tantric affairs of the temple.The festival days of the temple are Mahashivratri in the month of Kumbham and the Prethishtta day in the month of Idavam. On normal days, it is customary to open the temple only in the morning for pooja. Morning and evening poojas are performed only on Pradosha days. On the Navami and Dashami days of Navratri, VahanaPooja, PusthakaPooja and Vidyaramba are performed.
In the month of Karkitaka, Rudrabhishekam and Ganapati Homam are performed every day in the morning and Adhyatmaramayana Parayanam is performed in the evening. Dharamshastha is the sub deity of this temple.
